Based on interview and record review, the licensee did not comply with the section cited above. Child #1 did not have any immunizations. Child has been attending at the facility since 03/30/2024. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will contact Child #1's parents to inquire if immunizations will be given to Child #1, if parents still decline immunizations, child #1 will no longer attend at the facility. Licensee stated Child #1 will temporarily not attend at the facility starting 10/30/2024 until immunization are given. Licensee will email LPA plan for Child #1 by POC due date.
