Based on interview and record review, the licensee did not comply with the section cited above in 3 out of 3 persons( S1, S2, and S3) did not have updated Mandated Reporter Training Certificate. Missing documents were missing which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: S1, S2, and S3 will complete Mandated Reporter Training by due date.

Based on interviewre and record review, the licensee did not comply with the section cited above in 4 out of 4 persons were missing completed Infant Sleep Logs which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will subit Sleep Logs to LPA for one week to ensure licensee is completing Sleep Logs accurately.

Based on observation, interview, record review, Child #1 did not have a napping log, assistant stated they supervise the children while napping but they are not documenting every 15 minutes. the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will begin to document every 15 minutes on the napping log for child #1 and all infants that are under 24 months of age. Licensee will submit a copy of the napping log to licensing offie by the due date.

Based on observation, interview, record review, at 3:30pm while reviewing staff files licensee did not have a copy of the certificate and assistant had never completed the mandated reporter training. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will complete the mandated reporter online and send a copy to licensing office by the due date. LPA Odom provided the website www.mandatedreporterca.com (general and childcare).

Based on observation, interview, record review, at 3:30pm reviewing staff files, Staff #2 was missing a copy of their immunization records in their file. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated Staff #2 has a up coming dr appointment on 6/21/23 and they will obtain a copy of their immunizations records for MMR and TDap. Licensee will submit documentation to licensing by the due date.

Based on observation, interview, record review, at 3:00pm during children’s file reviews, LPA observed Child #1 was missing their immunization card in child’s file. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they had requested a copy of the immunization records but parents had not brought them. Licensee will request them today to obtain a copy. LIcensee will submit a copy of records to licensing office by the due date.

Based on observation, interview, record review, At 3:00pm during children’s file reviews, LPA observed Child #1 was missing LIC9227 Infant Safe Sleep plan. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will provide a copy of LIC9227 form to give to parent of Child #1 to complete and return the form. Licensee will submit a copy of the completed form to licensing by the due date.
