Based on record review, the licensee did not comply with the section cited above. During record review, licensee did not have a mandated reporter certificate for licensee and assistant. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she and assistant will complete mandated reporter trainings and will send the certificate to LPA by POC due date.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee did not have proof of immunization records for assistant. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will send a copy of immunization records for assistant to LPA by POC due date.

Based on observation, record review, during file reviews napping log was missing for all 3 infants under 24 months. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they were not aware of the new nappin log for infants up to 24 months. LPA Odom provided copies of form LIC9227 and napping log and reviewed it with the licensee. Licensee will submit a copy of the napping log for the infants by the due date.

Based on observation, record review, During staff file reviews licensee’s and assistant’s mandated reporter training certificate was expired. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will go online at www.mandatedreporterca.com to complete the childcare portion and renew their mandated reporter training certificate. Licensee will submit a copy of the certificate by the due date.

Based on observation, record review, During children’s file reviews inspection at 3:30pm LPA Odom observed child #6 and child#8 under 12 months were missing LIC 9227 Individual Infant plan. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will provide form LIC9227 to parents of C6 and C8 to complete and file in the child's file. Licensee will provide a copy to licensing office by the due date.
