Based on observation, the licensee did not comply with the section cited above in one out of one insecticide cans accessible to children, which poses/posed a potential health, safety or personal rights risk to persons in care. A can of insecticide was observed in the outdoor daycare area at a height of about 6ft. The licensee stated she had used it in the outdoor area and forgot to remove it once finished. The licensee was informed poisons must be locked.

Resolution: The licensee removed the can from the daycare area by throwing the can out. The licensee stated she will watch the video "Locks and Inacessibility Requirements in Child Care" located at https://ccld.childcarevideos.org/family-child-care-providers/locks-and-inaccessibility-regulations-in-child-care/ and send a declaration of completion to the LPA by the due date.

Based on observation and record review, the licensee (Vicky Benavides) mandated reporter training certificate expired on 3/6/2020. Licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will renew the childcare portion for the CA mandated reporter training online and provide a copy of the certificate to licensing office by the due date above. www.mandatedreporterca.com

Based on observation, interview, and record review, 1 out 2 children was missing the immunization records in the child's file. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will speak with child's parent today and request a copy of the immunization records. Licensee will submit a copy of the immunization records to the licensing office by the due date above.
