Licensee stated she was unaware a sleep log is needed for infants up to 2 years old. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will submit sleep logs for 3 out of 3 infants to LPA by POC due date.
Based on interview with Licensee Sandra Barahona, the licensee did not comply with the section cited above. Licensee Sandra no longer resides in the home and it was not reported to the office. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Sandra stated she will move back in the home to not be removed from the license, LPA informed Sandra that if she cannot move back in, to submit an application form to reflect the changes on the license.
Based on observation, interview, and record review, the licensee did not comply with the section cited above Uri Rubio has not completed fingerprint clearance and has been assisting for about 3 weeks. This poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will have Uri Rubio complete his fingerprints today and will not return until clearance is received.
