Based on LPA records review and Licensee interview, Licensee Sandra Olivio has been the new director for at least the past 6 months. Licensee stated she was not aware of submitting a Director's Packet to Child Care Licensing for review. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on LPA observation, LPA observed in North and South classrooms multiple damaged suspended ceilings from water leaks and normal wear and tear. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA observation, LPA observed in North and South classrooms multiple damaged suspended ceilings from water leaks and normal wear and tear. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
During the inspection, the LPA observed a child enter the bathroom by them self, and without anyone supervising. The child finished using the potty and washed their hands and then exited. The LPA had to point out to staff that the child was in the bathroom and no one noticed. This poses a potential health, safety or personal rights risk to persons in care.
No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced through statements collected through interviews with staff. Staff interviewed acknowledged a child was left unattended without supervision. This poses an immediate risk to the health, safety, and personal rights of children in care.
