Based on record review, the licensee did not comply with the section cited above in not providing the Department with a current mandated reporter training certificate for Licensee and all staff, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will send the department a copies of current mandated reporter training certificates for Licensee and all staff, by email by 10/31/25. Licensee stated she will add this reminder to her calendar to ensure this citation is not repeated.
Based on observation record review, Licensee and assistant S(1) and S(2) have not completed the Mandated Reporter Training, the licensee did not comply with the section cited above in that she, nor asssitants have not completed the Mandated Reporter Training course, which poses as a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee with written instructions on how to locate the Mandated Reporter Training online. Licensee states she and assistant S(1) and S(2) will complete the online course and provide LPA with a copy of the training completion certificates no later than 05/26/2023.
