Based on record review, the licensee did not comply with the section cited above in not providing the Department with a current mandated reporter training certificate, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will send the department a copy of a current mandated reporter training certificate by email by 10/31/25. Licensee stated she will add this reminder to her calendar to ensure this citation is not repeated.
Based on record review, the licensee did not comply with the section cited above in not providing the Department with safe sleep logs for Child #1, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will send the department a copy of a documented safe slepp logs by email by 10/31/25. Licensee stated she will ensure safe sleep logs are documented when infants are sleeping.
Based on observation, the licensee did not comply with the section cited above in having nine children (7 toddlers, 1 infant, and 1 school age), which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Two of the nine children were picked up by 1:50 PM. Licensee stated her helper was not able to come in to help today and will return tomorrow 10/7/25. Licensee stated she did not know what to do. Licensee signed a declaration stating she understands this regulation and will not happen again. Licensee stated if she does not have a helper she must follow reguation as small license ratios.
Based on interview and record review, the licensee did not comply with the section cited above in 6 out of 6 persons as Mandated Reporter training certification for licensee was not available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will complete the AB1207 Mandated Reporter training and provide proof of completion to LPA, no later than 5/22/2023.
