Based on record review, the licensee did not comply with the section cited above in Licensee did not provide documented drills and did not remember the last time they were conucted, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will send the department a copy of the documented drill by email by 8/13/25. Licensee stated she will provide the department a written statement on her plan of correction to ensure this citation is not repeated.

Based on record review, the licensee did not comply with the section cited above in not providing the Department with a current Mandated Reporter Training Certificate for Co-Licensee, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will send a current mandated reporter certificate for Co-Licensee by email by 8/29/25. Licenee stated she will provide the department a written statement on her plan of correction to ensure this citation is not repeated.

Based on interview and record review, assistant (S1) did not have Mandated Reporter training certificate on file for review, therefore co-licensee did not comply with the section cited above which poses as a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Co-Licensee with written instructions on how to locate the Mandated Reporter Training online. Co-Licensee states she will ensure assistant (S1) completes this online course and provides LPA with a copy of her training completion certificate no later than 04/10/2023.

Based on file review at annual inspection, co-licensee and assistant (S1) did not have the required immunization's (MMR and dTap) on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Co-licensee stated she will have herself and assistant (S1) obtain copies of required immunization's and send copies to CCLD by 4/21/2023.
