Based on record review, the licensee did not comply with the section cited above in not providing the Department with a current mandated reporter training certificate for Staff #2, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will send the department a copy of a current mandated reporter training certificate by email by 10/31/25. Licensee stated she will add this reminder to her calendar to ensure this citation is not repeated.

Based on record review, the licensee did not comply with the section cited above in not providing the Department with a current 1st Aid & CPR for Staff #1 and was alone with the children, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will send the department a copy of a current 1st Aid & CPR for Staff #1 by email by 10/10/25. Licensee stated she will add this reminder to her calendar to ensure this citation is not repeated.

Based on observation and record review, licensee's Mandated Reporter Training expired in 2018 and assistant S(1) has not taken the course, the licensee did not comply with the section cited above in that she has not renewed the Mandated Reporter Training course, nor assistant S(1) taken course which poses as a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Licensee with written instructions on how to locate the Mandated Reporter Training online. Licensee states she and assistant S(1) will complete this online course and provide LPA with a copy of her training completion certificate no later than 05/26/2023.

Based on interview and record review, assistant's S(1) immunization's were not available for review, the licensee did not comply with the section cited above in that one (1) out of (1) persons did not have immunization's records available which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain a copy of assistant's S(1) immunization's and send a copy to CCLD by 5/5/2023.

Based on analyst record review, the licensee did not comply with the section cited above in that three (3) out of three (3) children's files reviewed were incomplete and missing required documents and signatures which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: LPA provided Licensee the LIC311D which lists required forms for children's files. Licensee stated she will provide the three children's completed files to LPA by 5/12/23.

Based on record review, the Licensee did not comply with the section cited above in not having the required LIC 9227 Individual Sleeping Plan for infant in care which poses a potential health, safety to persons in care.

Resolution: LPA provided Licensee with a blank LIC 9227 form and PIN 20-24-CCP "Safe Sleep Regulations". The Licensee stated she will submit a copy of the completed LIC 9227 for the infants in care for the plan of correction to CCLD by 5/512/2023.
