Based on observation, a kitchen drawer containing knives/sharps did not have a lock, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the sharp items in LPA's presence. Deficiency is cleared today, however, licensee is advised to install a new child proof lock should she decide to use the drawer for knives/sharps. ***Failure to correct will result in a $100 per day civil penalty until corrected. Based on record review, three out of five children's files reviewed did not contain immunizations records and/or were not transferred onto the blue immunization cards, which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above as helper Elizabeth Claudio's mandated reporter training expired 11/28/23, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall ensure helper completes her mandated reporter training (child care provider and general training) and submit copy of certificate to LPA by due date of 2/20/24. ***Failure to correct will result in a $100 per day civil penalty until corrected.
