Based on observation, LPA observed an infant sleeping in crib while strapped with an infant swaddle, poses a potential health, safety or personal rights risk to persons in care. Swaddling is not permitted in child care.

Resolution: Licensee shall return the swaddle to the infant's parents and submit a statement to LPA ensuring compliance of this regulation. ***Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on interview and record review, the licensee did not have proof of a fire/disaster drill being completed, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: POC: Licensee shall conduct a fire/disaster drill and submit copy of log to LPA by due date of 4/19/24. ***Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
