Based on record review, S1 did not have proof of pertussis on file, and S2 did not have proof of measles, pertussis and TB clearance on file. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall submit proof of S1 (pertussis) and S2 (measles, pertussis and TB clearance) immunizations to LPA by due date of 7/25/25. ***Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on observation, the hot tub in backyard had a cover but it was not secured with a lock, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is advised to secure hot tub with a lock and submit proof to LPA by due date of 7/26/24. ***Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
