Both Licensee's Mandated Reporter trainings have expired as of 1/10/2025. Based on record review, the licensees did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees will ensure that the training is completed by end of day, 7/11/2025. Licensee will send LPA Pringle certificate of completion on or before POC date. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Licensee was observed to be out of ratio when assistant left the home at 12:20pm, leaving Licensee, Kumudini, alone in the home with 10 children. Based on observation, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees will develop a schedule that ensures that the facility remains in ratio at all times, including nap time. Licensee will send LPA the schedule on or before POC date. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Resolution: Licensee's will send LPA Pringle daily record of facility ratio's until POC date and statement explaining how the facility will remain in compliance with ratios moving forward.
