Additionally, licensee is advised to keep copies of sleep checks for 3 years for review by licensing. ***Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on record review, two children (C1 and C5) are missing the signed Affidavit Regarding Liability Insurance form, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, two infants were sleeping in pack-n-plays with blankets, which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, immunizations were not transferred for all children, which poses a potential health, safety or personal rights risk to persons in care.
