Based on record review, the licensee did not comply with the section cited above as her mandated reporter training is expired, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall complete the mandated reporter training (child care providers and general trainings) and submit certificate to LPA as proof of compliance by due date of 5/31/24. ***Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on record review, the licensee did not comply with the section cited above as C1 does not have a completed infant sleep plan (LIC 9227) on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall ensure parents of C1 completes an infant sleep plan (LIC 9227) and submit copy of form to LPA as proof of compliance by due date of 5/21/24. ***Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
