Based on observation, the licensee did not comply with the section cited above as infant's bottles/foods in refrigerator were not labeled with names and dates, which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, S4 does not have proof of measles and pertussis, which poses a potential health, safety or personal rights risk to persons in care.

.1(g)The licensee shall document each child's immunizations and shall maintain such documentation in the center for as long as the child is enrolled. (1) This requirement includes updating each child's immunization record when the child is due to receive required immunizations after enrollment in the child care center. This requirement is not met as evidenced by: Licensee did not document immunizations in 5 out of 6 files reviewed. This is a potential health and safety risk to child(ren) in care.

Based on observation, the licensee did not comply with the section cited above as there was Acetaminophen that expired 10/2022 and an Epi-pen that expired 4/2021. Both medications were not labeled with each child's name and date. This poses a potential health, safety or personal rights risk to persons in care.
