Resolution: Licensee shall submit proof of renewed mandated reporter training for child care providers by due date of 8/20/25.

Based on interview and record review, the licensee's CPR/First Aid training expired 6/10/25, which poses a potential health, safety or personal rights risk to persons in care. ***Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on record review, the last fire drill was logged on 8/22/23, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall conduct a fire drill and submit copy of log to LPA by due date of 8/26/24. ***Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on record review, the licensee did not comply with the section cited above as at least 3 children's files did not have immunization records transferred onto immunization cards (CDPH 286), which poses a potential health, safety risk to persons in care.

Resolution: Licensee shall ensure immunization records are transferred for all children in care, and submit copies to LPA by due date of 8/28/23.

Based on record review and interview, the licensee and helper do not have current mandated reporter certificates on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copies of mandated reporter certificates for herself and helper by due date of 11/7/22. Training may be taken at www.mandatedreporterca.com. Please take the training for "child care providers".
