Based on observation, 4 children were sitting outside the classroom door having snack without visual supervision from the teacher, who was inside classroom. This is an immediate health and risk to children in care.

Based on observation and record review, child C6's Epinephrine expired 9/21/23, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, the bathtub in outdoor play area at bottom of hill had about 6-8 inches of standing water inside, which poses potential health, safety or personal rights risk to persons in care.

Based on interviews and records review, EMSA approved CPR/First Aid cards were issued to staff without the required training. Former director advised staff that training was not needed due to COVID. This is a potential health and safety risk to children in care.
