Resolution: Licensee will start logging 15 minute sleep checks for all infants under 24 months and keep records at facility. LPA will review sleep logs through email or a follow up visit. Licensee will also train staff on how to document and the importance of safe sleep and sleep logs.

Resolution: Licensee will be required to provide a copy of her assistants' immuniation record by the above due date.
