Based on observation, interview, and record review, the licensee did not comply with the section cited above in not having sleep logs documented. Licensee was given a TV the year before for not doing logs, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will begin documenting 15 minute sleep checks for all infants under the age of 2 years old and educate staff on doing so as well daily. Licensee will send LPA proof by 9/19/25 after a months documentation.
Based on record review , the licensee did not comply with the section cited above in not having a valid Pediatric First Aid/CPR certification, one on file expired 1/2025, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will sign up for a Pediatric First Aid/CPR class and send LPA proof through email once complete by due date of 9/19/2025.
Based on interview, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: The licensee stated she would submit her Mandated Reporter Training certificate to the department by the due date mentioned above.
