Resolution: Licensee will complete the Mandated reporter training and send certificate to LPA a copy via email at sarah.garcia@dss.ca.gov by 5pm on 03/22/2024. LPA provided the Website: www.mandatedreporterca.com.
Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of the 5 children's files did not have immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain (1) child's immunization record and place in children's file. Licensee will send a copy of the (1) child's immunization record via LPA email at sarah.garcia@dss.ca.gov by 5pm 03/22/2024.
