Based on observation and record review, the licensee did not comply with the section cited above by using Bedroom #2 as a daycare area without notifing the Department which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall mail the following to LPA: LIC279 (check off "update"), a letter of intent stating which rooms will be added as daycare areas, an updated facility sketch (LIC999), and an updated emergency disaster plan (LIC610A).
Based on observation, the licensee did not comply with the section cited above in not having a present assistant with a large licensed capacity which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Per licensee, assistant was out sick and back up assistant was on vacation. Licensee will ensure to have present assistant or operate as a small license.
