At 10:00AM., Based on Record Review, LPA confirmed staff's proof of mandated reporter training (AB1207) missing from 2 of 3 facility records. This poses a potential risk to the health and safety of children in care.

Resolution: Facility Assistant (S2, S3) will complete the required mandated reporter training course (AB1207) by the due date: 10/2/2025. Proof of corrections will be submitted to LPA via email.

At 10:55AM., Based on record review and interview, LPA confirmed licensee not maintaining proof of 15 minute review during infant nap. This poses a potential risk to the health and safety of children in care.

Resolution: Staff will begin documenting infant napping contions, in 15 minute intervals, for infant-age children in care by the due date: 9/25/2025. Proof of correction will be submitted to the LPA via email.

Based on record review, the licensee did not comply with the section cited above, no staff present have their Madated Reporter Certification which poses a potential health, safety or personal rights risk to persons in care.

At 2:20PM., Based on record review, interviews, LPA confirmed licensee is over capacity stated on license with four infants (13 children) in care. This poses an immediate health and safety risk to children in care.

Resolution: Licensee will reduce infant enrollment by the due date: 12/26/2022 Updated schedule/ roster (LIC9040) and updated schedule will be submitted to the Department via email. Registered parents must sign the LIC9224.

At 2:40PM., Based on record review and interview, LPA confirmed employee present, S1, without proper fingerprint clearance on file. This poses a potential health and safety risk to children in care.

Resolution: Licensee will ensure helper,S1, receives proper criminal record clearance prior to presence in facility. Proof of correction will be submitted to the department via email.

At 2:30PM., Based on record review, LPA confirmed licensee is not maintaining documentation of infant napping conditions every 15 minutes. This poses potential health and safety risk to children in care.

Resolution: Licensee will maintain documentation of napping conditions for infants in care by the due date:12/30/2022. Proof of correction will be submitted to department via email.
