Based on interview and record review, the licensee did not comply with the section cited above by not logging infant sleep checks which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall maintain infant sleeping logs for all infants (0-24 months) in care. A sample sleep log was provided. A follow-up visit shall be conducted to verify compliance.

Based on interview and record review, the licensee did not comply with the section cited above by not maintaining complete personnel records for all staff present which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall maintain a complete file for all staff members present. LIC126 (Entrance Checklist) was provided for reference. A follow-up visit shall be conducted to verify compliance.

Based on record review, the licensee did not comply with the section cited above by not maintaining a file for each child which contains an emergency information card which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that a complete file shall be maintained for each child in care which contains an emergency information card as specified above. A follow-up visit shall be conducted to verify compliance.

Based on interview and record review, the licensee did not comply with the section cited above by not conducted an emegrency drill at least once every six months, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall conduct an emergency drill and maintain documentation of the drill accessible for review. Proof of completion shall be sent to LPA via email.

Based on observation, the licensee did not comply with the section cited above by not keeping two out of three cribs free of loose articles and objects which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed all loose articles and objects from the cribs during the visit. A follow-up visit shall be conducted to verify compliance.

Based on observation, the licensee did not comply with the section cited above by not having a helper or provider stationed in Bedroom #3 with two napping infants, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall ensure that a helper or provider is present with napping infants. If no helper or provider is available, the door must remain open such that napping infants can be seen without moving the door. A follow-up visit shall be conducted to verify compliance.

Based on record review, the licensee did not comply with the section cited above in not having any immunization on file for staff and herself which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee plans to locate immunization records for herself and H1 then send it to the department and place in staff files.

Based on record review, the licensee did not comply with the section cited above in not having current Mandated Reporter Certificates which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee plans to retake training and send certificates to LPA within next 10 buissness days.
