Based on observation, interview, and record review, the licensee did not comply with the section cited above in not having LIC 9227 in infants' file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will have parent to fill out and signed the form and place them in the file by next week and provide a proof to the LPA via email.

Based on observation, interview and record review, the licensee did not comply with the section cited above in not having valid Mandated Reporter training certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated if she cannot find the certificate tomorrow she will retake the training and provide a copy of the certificate to the LPA via email on 2/24/2025.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in not providing the records to Licensing Staff to review during the inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will have her assistants to obtain the immunization records by 2/28/2025.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in not having a valid CPR certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will provide certificate to the LPA upon completion.

Based on observation interview, and record review, the licensee did not comply with the section cited above in not checking infants during everyday nap time which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will ensure all assistants and herself maintain 15 15-minute sleep check starting tomorrow, 2/21/2025.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in having an unfingerprinted adult in the facility which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee will provide a copy of LIC 9163 form to the LPA via email by tomorrow, 2/21/2025.

Based on observation, interview, and the record review, the licensee did not comply with the section cited above in having children more than the facility's capacity which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee will ensure to be following the capacity regulation, starting tomorrow, 2/21/2025. She will make sure the tenant does not have more than 2 children starting tomorrow.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in having a baby walker in the daycare during daycare hours which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee removed the baby walker outside the facility to give it away during the inspection.
