Based on record review, the licensee did not comply with the section cited above in five out of five personnel records which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall acquire immunization records for all adults working with children in the home and maintain them in the personnel files. Licensee shall submit proof of completion by submitting copies of immunization records to LPA by set due date of 1/24/2024.
Based on observation, interview and record review, the licensee did not comply with the section cited above by not having an emergency identification card for any enrolled child, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA reviewed list of documents that need to be maintained for all enrolled children. Licensee to update children's records to have an emergency information identifcation card for each enrolled child. Licensee to provide LPA proof of updated children's records no later than September 9, 2022 by 5:00pm.
