Based on record review, the licensee did not comply with the section cited above in 5 out of 10 children's files, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall maintain documentation of the required files in each child's file. Proof shall be submitted to LPA via email.

Based on observation, the licensee did not comply with the section cited above by not having operational carbon monoxide detectors present in the home, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall purchase and install new and operational carbon monoxide detectors. Documentation such as receipts and photos of installed detectors shall be sent to LPA via email.

Based on observation, the licensee did not comply with the section cited above by not securing drawers and stove covers, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Stove covers and childproof locks or latches shall be installed on kitchen cabinets, drawers and the stove as necessary. Photos shall be submitted to LPA via email.

Based on interview and record review, the licensee did not comply with the section cited above by not documenting emergency drills, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: An emergency drill shall be conducted and a photo of a documented log shall be submitted to LPA via email.

Based on record review, the licensee did not comply with the section cited above in 1 out of 2 staff files, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall obtain documentation of H2's immunizations and submit documentation to LPA via email.

Based on record review, the licensee did not comply with the section cited above in maintaing all immunization records for staff which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided all needed immunization information for staff. Per licensee she will have her staff Manana Sadgobelashvili complete/locate all required immunization, place it on file. and email LPA Gil.

Based on record review, the licensee did not comply with the section cited above in not having valid Mandated Reporter Training avalible to the department for herself and assitant which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per lisencee, she will complete Mandated Reporter Training by the end of the day on 4/3/2024 and send it to LPA Gil via email.Her assitant plans to do the same.

Based on record review, the licensee did not comply with the section cited above in not having avalible immunization records of her staff for the department which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Lisencee plans to obtain assitant's immunization records and send them to LPA Gil via email by next week.

Based on observation, interview, and record review, the licensee did not comply with the section cited above with A2, an uncleared individual, directly working with and supervising enrolled children, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will either have A2 go through live scan process to receive fingerprint clearance or licensee will release A2 from work at facility. LPA to conduct follow up inspection.
