Based on observation, interview, and the record review, the licensee did not comply with the section cited above as she did not have a valid CPR+First Aid certificate which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will contact the provider to enroll the class for CPR training. The licensee will provide a proof of enrollment by 1/23/25 and the certificate upon completion to the LPA via email.
Based on record review, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee shall have helper H2 fingerprinted and submit proof of fingerprinting to LPA by end of day, 1/18/2024. Licensee shall submit LiveScan receipt in addition to LIC9163 that will be provided to the LiveScan vendor to LPA.
Based on record review, the licensee did not comply with the section cited above in one out of three personnel records which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall have maintain the following documents for all adults working with children in the facility: Mandated Reporter Training, LIC9108, immunization records, TB clearance, and LIC9052. Licensee shall submit proof of completed personnel records to LPA by due date of 1/31/2024.
