Based on observation, the licensee did not comply with the section cited above. LPA observed and infant asleep with a boppy pillow and blanket in a play yard which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee's assistant removed items during visit. LPA requested licensee and assistant Tasha Duarte read Safe Sleep regulations and submit a statement stating they understand the regulations.

Based on observation, the licensee did not comply with the section cited above. LPA observed infant with a pacifier in it's mouth with beaded chord attached to it which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA observed chord was removed from pacifier. LPA requested licensee and assistant Tasha Duarte read Safe Sleep regulations and submit a statement stating they understand the regulations.

Based on interview, the licensee did not comply with the section cited above. Licensee's son Michael Singleton does not have criminal record clearance which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee understands that her son Michael Singleton May not be in the home until criminal record clearance is obtained.

Based on observation, the licensee did not comply with the section cited above. Present were Tasha and nine day care children including eight preschool age children and one infant. Licensee was over capacity which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement stating how she will remain in capacity to CCLD by POC date.

Based on interview, the licensee did not comply with the section cited above. Licensee did not inform the Department that son Michael is living in the home. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit an updated LIC279 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. LPA observed child 2 did not have LIC9227 on file at time of enrollment which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement stating she understands the above regulation to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Assistant Tasha Duarte has not completed Mandated Reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit an updated Mandated Reporter Training Certificate for assistant Tasha Duarte to CCLD by POC date.

Based on interview, the licensee did not comply with the section cited above. Upon arrival, licensee stated an infant was asleep in an off limits room within the home which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's assistant removed child during visit. Licensee understands she may not use any off limits rooms unless approved prior by the department.

Based on record review, the licensee did not comply with the section cited above. Child 5 and 10 immunization records need to be updated which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit current immunization records for child 5 and 10 to CCLD by POC date.
