Based on record review, the licensee did not comply with the section cited above in 4 out of 5 children do not have completed consent for emergency medical treatment forms, LIC627, at the facility. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will have the parents of C2-C5 complete the LIC627. The licensee will send LPA copies of the completed document via email by 1/6/23.

Based on record review, the licensee did not comply with the section cited above in 3 out of 5 children do not have verification of immunizations at the facility. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will have the parents of C2, C3 and C5 submit verification of their child's immunizations. The licensee will send LPA copies of the documents via email by 1/6/23.

Based on record review and licensee statement, the licensee did not comply with the section cited above in 3 out of 3 infants do not have sleep documentation available for review at the facility. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will complete a sleep log for the infants (C2, C3 & C5) and send LPA copies of the completed documents via email by 1/13/23.

Based on record review and licensee statement the last disaster drill occurred on 2/24/22. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will conduct and document a disaster/fire drill. The licensee will send LPA a copy of the documentation via email by 1/13/23.

Based on record review and licensee statement, the licensee did not comply with the section cited above. The licensee does not maintain CPR certification that meet regulations pursuant to Heath & Safety Code. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will take a CPR/First Aid course approved by the Emergency Medical Services Authority (EMSA) which contains an in person skills assessment. The licensee will send LPA a copy of the completion certificate via email by 1/31/23.
