Resolution: The licensee states that she will have Anthony Pinder have has fingerprints taken by 5/19/23 to obtain a criminal record clearance. The licensee also states that she will send LPA a copy of the LiveScan receipt as verification that Mr. Pinder's fingerprints have been submitted by 5/19/23.
Based on record review and licensee statement, the licensee did not comply with the section cited above. The licensee and helper Yolanda Perales do not have current mandated reporter certification. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee states that she and helper Yolanda Perales will take mandated reporter training and she will send LPA a copy of the completion certificates via email by 5/26/23.
