(1) For a license issued on or after July 1, 2022, initial testing results shall be received and posted within 180 days of licensure.

Based on interviews conducted by LPA, on 1/18/23 the parent/guardian of a daycare child (C1) was not immediately notified that the child was ill & vomiting in a classroom. This poses a potential health and safety risk to the children in care.
Based on interviews conducted by LPA, S1 picked up a child (C1) by one arm and one leg to move the child while upset with the child and S2 grabbed/pulled another child (C2) by the arm/wrist to move the child. This poses a potential health, safety or personal rights risk to the children in care.
