Based on interview and record review, the licensee did not comply with the section cited above in that Licensee could not provide a current Mandated Reporter Training Certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will submit proof of Mandated Reporter Training certificate to the Dept by 12/1/25.

Based on observation, the licensee did not comply with the section cited above in that medicine was observed accessible to children in the bathroom medicine cabinet. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will remove the medications and hazardous items from the bathroom medicine cablinet. The licensee will send LPA a picture of the cabinet cleared of medication/hazardous items via email by 11/10/22.

Based on record review, the licensee did not comply with the section cited above. The licensee and helpers Analy and Miguel Gaytan do not have current mandated reporter certificates available for review. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will submit current mandated reporter certificates for herself and helpers to LPA via email by 11/17/2022.

Based on record review & licensee statement, the licensee did not comply with the section cited above. The licensee does not maintain verification of immunization to measles and pertussis for herself and helpers (Miguel Gaytan and Analy Gaytan) at the facility. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will submit verification of measles and pertussis immunization for her helpers Miguel and Analy Gaytan and herself via email by 11/30/22.

Based on record review, the licensee did not comply with the section cited above in 2 children (C7 & C8) are missing consent for emergency medical treatment forms (LIC627) and one child (C8) was missing the ID and emergency information form (LIC700). This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will submit the missing documents to LPA via email by 11/10/22.

Based on record review, the licensee did not comply with the section cited above. The licensee does not maintain a current children's roster. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will create a children's roster (LIC9040) and send LPA a copy of the document via email by 11/10/22.

Based on record review, the licensee did not comply with the section cited above in 5 out of 9 children's files reviewed do not contain the LIC995A. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will have the parents/guardians of the 5 children missing the LIC995A conplete the document. The licensee will send LPA copies of the signed & dated LIC995A's via email by 11/10/22.
