Based on Director and staff interviews the licensee did not comply with the section cited above in that Child #1 was left unattended for approximately under one minute without staff supervision which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on staff statements, staff did not use the existing 2-step protocol in place to make sure the correct baby bottle was given to the correct infant. This posed a potential risk to the health and safety of infants in care.

(1) The center shall give each authorized representative a copy of the Personal Rights form (LIC 613A [9/96]).
(a) The written infant needs and services plan shall be updated at least quarterly, or as often as necessary to assure its accuracy.
(3) Prescription medications may be administered if all of the following conditions are met: (A) Prescription medications shall be administered in accordance with the label directions as prescribed by the child's physician.
