Based on observation, the licensee did not comply with the section cited above. LPA's observed an infant standing in a crib with a pillow and blanket laying on the bottom of the crib. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee removed the blanket and pillow from the crib at the time of inspection.

Based on record review, the licensee did not comply with the section cited above in 1 out of 6 children's files reviewed did not contain verification of immunizations. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will obtain immunization records for C1, and she will send a copy of the records to LPA Curtis via email by 5/31/23.

Based on record review, the licensee did not comply with the section cited above in 1 out of 1 infant under 12 months did not have an Infant Sleeping Plan, LIC9227 available for review. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will send LPA Curtis a copy of the completely LIC9227 for C1 via email by 5/31/23.

Based on record review and licensee statement, the licensee did not comply with the section cited above. The licensee does not have a current children's roster available for review. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will submit a children's roster to LPA via email by POC due date of 2/24/22.

Based on record review and licensee statement, the licensee did not comply with the section cited above. The last documented disaster/fire drill occurred on 2/10/21. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will conduct and document a disaster/fire drill. The licensee will send LPA a copy of the documentation via email by POC due date of 2/17/22.

Based on record review and licensee statement, the licensee did not comply with the section cited above in three out of three infants do not have sleep documentation/log available for review. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will create a sleep log/document for the three infants present during the inspection and forward a copy of the completed document to LPA via email by POC due date of 2/24/22.

Based on record review, the licensee did not comply with the section cited above. The licensee and helpers Charlie Abayan and Alfeia Termulo do not have current mandated reporter certification. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she and her helpers will complete mandated reporter training. The licensee will submit the completion certificates to LPA via email by POC due date of 2/24/22.

Based on record review, the licensee did not comply with the section cited above in one out of ten children do not have immunization records at the facility. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will forward a copy of the child's missing immunization record to LPA via email by POC due date of 2/24/22.
