Based on observation, the licensee did not comply with the section cited above in that the home has a fire extinguisher that was last serviced on 3/31/23 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of a newly serviced or purchased fire extinguisher to CCLD (shane.loftus@dss.ca.gov) by 4/10/25.

Based on record review, the licensee did not comply with the section cited above in that the licensee does not have documentation of fire drills conducted at the Family Child Care Home. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of documentation of a fire drill conducted within the last 6 months to CCLD (shane.loftus@dss.ca.gov) by 4/10/25.

Based on record review, the licensee did not comply with the section cited above in that licensee does not have current Mandated Reporter training (AB1207) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of current Mandated Reporter training (AB1207) to CLLD (shane.loftus@dss.ca.gov) by 4/10/25.

Based on observation, the licensee did not comply with the section cited above in that the licensee did not have PUB 394 posted in a prominent, publicly accessible place. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of PUB 394 (Parents' Rights) posted in a prominent, publicly accessible area to CCLD (shane.loftus@dss.ca.gov) by 4/10/25.

Based on record review, the licensee did not comply with the section cited above in that the licensee did not have emergency information on file for C1, this poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of a LIC 700 (ID and Emergency Contact Information) LIC627 (Consent for Emergency Medical Treatment) for the child in question to CCLD (shane.loftus@dss.ca.gov) by 4/10/25.

Based on record review, the licensee did not comply with the section cited above in that C1 does not have a 995A (Parent's Rights) on file, which poses a potential health, safety or personal rights risk to persons in care.
