Resolution: The licensee stated she will remove the baby bouncer from the on-limit area of the home immediately.
Based on record review, the licensee did not comply with the section cited above the assistant did not have her immunization records (MMR) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain the immuization records for her assistant and send to LPA Woods by 09/11/2024 by regular mail, fax, or text. LICENSEE SHALL OBTAIN THE IMMUNIZATION RECORDS FOR THE CHILDREN AND SEND LPA A COPY

Resolution: Licensee will immediately begin documenting the time, date, and condition observed of each infant during each 15-minute interval of sleep. Infant safe sleep logs will begin to be kept and retained in children’s files for 3 years as of 08/04/2023.
