Based on observation, interview, and record review, the licensee did not comply with the section cited above. During record reviews, LPA observed last emergency evacuation drill was logged on 10/20/2021 which poses/posed a potential health, safety or personal rights risk to persons in care.

During staff file reviews, LPA Observed 5 out of 6 staff files were missing their Mandated Reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.

While touring the facility, LPA observed Staff #1 caring for eight children inside classroom #117. LPA observed Staff #1 did not obtain Criminal Record Clearance prior to working in a license facility. LPA observed Staff #1 Determination Status was still in progress which poses an immediate health, safety or personal rights risk to persons in care.

Based on LPA's interviews and record review. LPA interviewed 11 staff 2 staff stated they herd (S9) handle C1 in a rough manner. 2 staff stated it was reported to them.Records review showed facility met with employee and Employee Counseling Notice for (S9)was created.
