Based on record review, the licensee did not comply with the section cited above in that C2 and C5 did not have the LIC 995A signed and in their file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will obtain a signed copy of the LIC 995A for C2 and C5 and send LPA a copy by due date. LPA provided Licensee with a copy of the FCCH entrance checklist.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in that S2 did not have their child care criminal record clearance completed and S2 is not associated to the facility which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LPA observed S2 leave the facility at approximately 10:18AM. Licensee stated that 10/28/2025 is S2's last day at the facility.
Based on interview and record review, the licensee did not comply with the section cited above in that the licensee did not have documentation showing fire/disaster drill are being conducted and documented at least once every six months which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will document their disaster drills and submit a copy to LPA by due date.
Based on interview and record review, the licensee did not comply with the section cited above in that the Licensee and S3 did not have their mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she and S3 will complete their mandated reporter training and send LPA a copy of the certificate by the due date.
Based on record review, the licensee did not comply with the section cited above in that S3 did not have a copy of their immunizations which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will obtain a copy of S3's immunizations and send LPA a copy by due date. LPA provided Licensee with a copy of the FCCH entrance checklist.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Upon arrival at 9:15 a.m. LPA observed Danaan Wilson caring for seven children in the outdoor play area. LPA observed Danaan Wilson has not submitted fingerprints prior to working in a licensed facility which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LPA observed Danaan Wilson leave the family child care home during the inspection.
Based on interview and record review, the licensee did not comply with the section cited above. Licensee was unable to provide proof showing she is condcuting and documenting fire/disaster drill at least once every six month which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit proof of documentation to LPA by POC due date.
Based on interview and record review, the licensee did not comply with the section cited above. Licensee and assistants did not have proof of their mandated reporter certifications which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to conduct the mandated reporter training and have assistants conduct the training as well then submit proof to LPA by POC due date
Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed assistants were missing immunization against pertussis and measles as well as tuberculosis which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to update assistants file and send proof to LPA by POC due date
Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed 5 out of 7 children present were missing immunization records from their file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to obtain immunization records for the five children and send proof to LPA by POC due date.
