Based on record review of licensee and staff S1, 1st aid/CPR were expired which poses a potential health, safety or personal rights risk to persons in care.

Resolution: By POC due date 1/31/2025, Licensee will provide copy of current 1st aid/CPR certificates.

Based on record review for child C5 file is missing 15 minute sleep check log, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: By POC due date 1/10/2025, licensee will provide copy of 15 minute check log.

Based on record review for licensee, and staff S1 & S2 were missing or had expired mandated Reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: By POC due date of 1/15/2025, licensee will provide copy of completed Mandated Reporter training certification.

Based on record review for staff S2, file was not available at time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: By POC due date 1/24/2025, Licensee will provide a copy of staff S2 record and immunization.

Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee and adult assistants present during inspection did not have proof of current Pediatric First Aid and CPR training which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit to LPA proof of Pediatric First Aid training certificate by POC date.

Based on observation, interview and record reviews, the licensee did not comply with the section cited above. Licensee did not have child's file for Child 5 and Child 9 which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit Child's file for Child 5 and 9 that include all required information on the emergency information card.

Resolution: Licensee will submit to LPA child's file for Child 5 and Child 9 which includes the copy of the LIC995 Notice of Child Care Home Notification of Parent's Rights by POC due date.
