Based on observation and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: By the Plan of Correction (POC) due date 04/25/2025, Licensee will submit an updated fire and disaster drill log to the department as a proof of correction.

Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: By the Plan of Correction Due date 04/25/2025, Licensee will submit the current Mandated Reporter Training Certificate of the Licensee, and three adult assistants to the department as a proof of correction.

Resolution: By the Plan of Correction (POC) due date 04/25/2025, Licensee will submit the copy of immunization record of the children C1 and C2, as a proof of correction to the department.

Based on observation, interview and record review, the licensee did not comply with the section cited above. The Licensee, her spouse and two adult assistants did not have a current Mandated Reporter Training Certificate upon inspection which pose a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit Mandated Reporter training certificates for herself, her spouse and two adult assistants by POC date 6/4/22.

Based on observation, interview and record review the licensee did not comply with the section cited above. Licensees' adult assistants did not have personnel records readily available for licensing agency to review which pose a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit and maintain complete personnel records as required but not limited to: LIC508Criminal Record Statement, LIC9052 Employee Rights, Proof of Immunizations (measles, pertussis, flu), Tuberculosis clearance, LIC 9108 Statement Acknowledging Requirement to Report Child Abuse for all adult assistants providing care to children in her daycare.
