Based on record review, the licensee did not comply with the section cited above. LPA observed a safe sleep log for infants in care but has not been updated since 03/25/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit Safe Sleep log for infants in care for week of 04/15/2024 - 04/19/2024 to CCLD by POC date.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee and assistants do not have completed Mandated Reporter certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a current Mandated Reporter certificate for her and her assistants to CCLD by POC date. Training can be found at www.mandatedreporterca.gov.

Based on record review, the licensee did not comply with the section cited above. Assistant Irma is missing measles and influenza immunization and Maribel is missing measles, influenza and pertussis which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit immunizations for assistants to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 10 is missing LIC627 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC627 for child 10 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 10 is missing LIC995 which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above. Yvonne Vargas does not have criminal record clearance and was alone in the backyard playground with eight children which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee had Yvonne go home and understands she may not return until she obtains criminal record clearance.

Based on record review, the licensee did not comply with the section cited above. LPA observed a gas powered lawn mower in the backyard were children were playing and toothpaste on the bathroom counter accessible to children which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed during visit. Deficiency cleared today.

Based on record review, the licensee did not comply with the section cited above. LPA observed a fire and disaster drill log was last completed on 04/20/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire and disaster drill and submit updated log to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA observed a baby bouncer in the back yard which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed during visit. Deficiency cleared today.

Based on record review, the licensee did not comply with the section cited above in Child 08, 12, and 13 do not have LIC 995A signed which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain parent signatures for form LIC 995A and will submit a copy of the completed form to the San Jose Regional Office (SJRO) by close of business on June 15, 2022

Based on record review, the licensee did not comply with the section cited above in which Child 01, 04, and 07 are missing 15 minute sleep logs which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will create and document infants sleep log and will document sleep every 15 mintues for all infants under 24 months. Licensee will send copies of completed logs to SJRO by close of business on June 15, 2022.

Based on record review, the licensee did not comply with the section cited above in which Child 08 is missing signatures on LIC 627 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain signatures on LIC 627 and will submit a copy of completed form to SJRO by close of business on June 15, 2022

Based on record review, the licensee did not comply with the section cited above in which licensee was unable to provide a certificate of completion for Mandated Reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they have taken the class, but have not received the certificate. Licensee will obtain a certificate of completion from the union and will send a copy to SJRO by close of business on June 15, 2022
