Based on observation, the licensee did not comply with the section cited above in, LPA observed a loose fitting sheet inside the portable crib inside the day care room while an Infant was asleep, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the sheet and stated that she will replace the sheet that fits tightly on the mattress and overlaps the underside of the mattress so it cannot be disloged.
Based on observation, the licensee did not comply with the section cited above iin, LPA observed a mobile hanging over the portable crib inside the livingroom, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the mobile and stated that she will not have any mobile or any other objects hanging above the cribs.
Based on observation and interview,the licensee did not comply with the section cited above in, LPA was unable to review/observe a 15 minute check for C1,] which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will submit a plan to ensure that she will document 15 minutes checks for infants and to submit to CCL by the POC date.
Based on observation and record review, the licensee did not comply with the section cited above in, C1 did not have an Individual Infant Sleeping Pan (LIC9227) on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a completed C1's Individual Infant Sleeping Plan(LIC9227) to CCL by the POC date.
