Resolution: By POC due date of 7/11/2025, Licensee will provide proof of immunization record for child C2.

Based on interview, and record review], child C1 was missing infant sleep plan LIC 9227 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: By POC due date of 7/11/2025, Licensee will provide copy of infant sleep plan LIC 9227 for child C1.

Based on observation and interview, the licensee did not comply with the section cited above. LPA observed licensee swaddle infant before placing them to sleep which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee, Narumi Tashiro will review Childcare Regulations on Safe Sleep and submit a Plan of Correction to LPA by POC due date of 08/12/2023.

Based on observation, record review and interview, the licensee did not comply with the section cited above. Last fire drill was conducted and logged on 5/14/2021 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: License will conduct and log fire drills every 6 months. Licensee will submit proof of fire drill to LPA by POC due date of 08/25/2023

Based on observation and record review, the licensee did not comply with the section cited above. Licesee does not have a current Mandated Reporter Training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete the online Mandated Reporter TRaining and submit certificate of completion as proof to LPA by plan of correction date of 08/25/2023.

Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee does not have a current pedriatric CPR/first aid certification which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete and EMSA certified Pediatric CPR/First Aid training and submit proof of completion to LPA by plan of correction date of 08/25/2023.

Based on observation, interview and record review, the licensee did not comply with the section cited above for one child which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain immunization record for one child and submit proof to LPA by plan of correction date of 08/25/2023.

Based on observation., interview and record review, the licensee did not comply with the section cited above in one out of one infant which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have parents/authorized representatives complete required form and submit proof to LPA by plan of correction date of 08/25/2023.

Based on observation, the licensee did not comply with the section cited above. LPA observed garden fertilizer and sanitizer near the front entrance within children's reach. LPA observed mouth wash, lens solution, lysol wipes and other cosmetics under bathroom sink which is not secured with a child lock and garden tools and hammer in the backyard in areas accessible to children which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee, Narumi Tashiro will submit a plan of correction to LPA by POC due date of 08/12/2023.

Based on observation and interview, the licensee did not comply with the section cited above. LPA observed two baby bouncers in the on limit family room and Licensee placed infant in bouncer while LPA was present. This poses an immediate health, safety or personal rights risk to persons in care. Licensee placed the bouncers in an off limit area while LPA was present.

Resolution: Licensee, Narumi Tashiro will review Childcare Regulations and submit a Plan of Correction to LPA by POC due date of 08/12/2023.
