Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During today's inspection LPA observed a bottle of Lysol Mold and Mildew cleaner that was placed in the right side area. Licensee stated that the area is off limit and children do not use the area but LPA did not observed a barricade/gate making the area off limits. Licensee placed the bottle of cleaner in the shed during today's inspection.

Resolution: Licensee to submit a written plan of correction to LPA by plan of correction due date of 08/20/2025.

Based on observation,interview and record review, the licensee did not comply with the section cited above for all 3 children which poses a potential health, safety or personal rights risk to persons in care. LPA observed no LIC 9224 with parents' signatures for the type A deficiencies cited on 07/29/2024 in all children's files.

Resolution: Licensee will provided parents with facility inspection report along with deficiencies cited on 07/29/2024 and have them sign the LIC 9224. Licensee will submit a copy of signed LIC 9224 for three children to LPA by 08/20/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above for Licensee, Tuyet Mai which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to complete the training and submit certification of completed training to LPA by plan of correction due date of 08/20/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above for both Licensees which poses a potential health, safety or personal rights risk to persons in care. Both Licensee's do not have current Mandated Reporter Training's. LPA reminded licensee that Mandated Reporter Training needs to be renewed every two years. Based on observation and interview, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. During the facility walkthrough, LPA observed a can of insect killer and gardening tools near the outside deck accessible to children and several bottles of cleaning products in the on limit bathroom that are not made inaccessible to children.

Resolution: Licensee placed insect killer and cleaning products inaccessible during today's inspection. Licensee to submit a written plan stating understanding of regulations to LPA by POC date of 07/30/2024.

Based on observation, interview and record review, the licensee did not comply with the section cited above for one child (C6) which poses a potential health, safety or personal rights risk to persons in care. Licensee did not maintain the required forms for Child, C6.

Resolution: Licensee will have parents or authorized representatives complete the required forms and submit proof to LPA by POC date of 08/12/2024.

Based on observation and interview, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee needs to purchase new fire extinguisher or have the fire extinguisher recharged. Licensee will send a picture of the new fire extinguisher and receipt as proof of purchase by Plan of Correction date of 07/10/2023.

Based on observation, record review and interview, the licensee did not comply with the section cited which poses a potential health, safety or personal rights risk to persons in care. Licensee did not have a current facility roster.

Resolution: Licensee will submit a completed facility roster to LPA by Plan of correction date of 07/10/2023.

Based on observation, record review and interview, the licensee did not comply with the section cited above for 5 out of 5 children which poses a potential health, safety or personal rights risk to persons in care. Licensee did not have required forms/files for all children in care.

Resolution: Licensee will submit proof of all required forms for five (5) children in care by Plan of Correction Date of 07/10/2023

Based on observation and record review, the licensees did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. Licensees Vinh Ta and Tuyet Mai, do not have a current Mandated Reporter Trainings.

Resolution: Licensees will complete online training and send Certificate of Completion as proof by Plan of Correction date of 07/10/2023.

Based on record review and interview, the licensee did not comply with the section cited for one adult residing in the home which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will have adult son residing in the home fingerprinted and submit proof of clearance and association to LPA by Plan of Correction date of 06/27/2023.
