LPA was greeted by the Director Neda Ghahghani and provided access to the facility. LPA stated the reason for the visit was to investigate the self reported incident that occurred on 02/03/2026. LPA toured the facility and collected the census. There were 96 preschool age children and 15 staff (14 Teachers and 1 Director) present which is compliant with the ratio and capacity requirements. LPA conducted interviews, observed the classroom transition and reviewed relevant documentation. Based on the facility's written report, it was confirmed that Child #1 was left unattended without staff for 10 seconds during the transition from bathroom to the classroom. The child was inside the facility grounds which is gated and secured. This is a zero-tolerance violation. Due to today's inspection, a Type A Citation has been cited along with a Civil Penalty.
