During record review LPA observed facility does not have a designated fully qualified teacher during all operational hours in the absence of the Director. Staff responsible to act in Director’s absence as indicated on the Designation of Facility Responsibility (LIC 308) are not present all hours the center is in operation.

Based on record review, the licensee did not comply with the section cited above by not having Staff 4 (S4) re-new her mandate report training that expired on 10/26/2024 which poses a potential health, safety or personal rights risk to persons in care.

(g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children.

(1) The center shall give each authorized representative a copy of the Personal Rights form (LIC 613A [9/96]).

During facility walkthorugh LPA observed one child using one stall in the bathroom and staff (S1) standing outside the bathroom door and not maintaining visual supervision. Based on staff interviews, staff stated that they have observed children using the restroom with staff positioned away from the restroom.
