Based on record review, the facility did not comply with the section cited above as child (C1) and child (C2) do not have LIC 9227 in file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The facility will ensure that all children under the age of 12 months shall have the LIC 9227 completed and on file for review.

Based on record review, the facility did not comply with the section cited above staff (S1) did not have proof of tuberculosis test and measles, mumps and rubella vaccination. (S3) did not have proof of tuberculosis test which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The facility will ensure that all required immunization are on file for staff.

Based on record review, the licensee did not comply with the section cited above in 2 out of 5 child files is missing updated immunization records or using the PM 286 form which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The facility will ensure the two infant children have their current immunizations records.

Based on observation, the facility did not comply with the section cited above as staff were in the play area and could not visually supervise the children and do not conduct 15 minute checks the licensee which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The facility will ensure they conduct physical checks of sleeping infants to observe for labored breathing, signs of distress and sleeping position

Based on record review and observations, the facility did not comply with the section cited above as the staff did not check on the children every 15-minutes or document any checks while the infants were in the play yards which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The facility will ensure they conduct 15-minute checks and document their observations for each infant.

Based on observation, the facility did not comply with the section cited above as the carbon monxide and smoke alarm were inoperable which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The facility will ensure the smoke and carbon monoxide detector are operable at all times.

Based on observation, the facility did not comply with the section cited above by cleaning compounds located in the kitchen which is on-limit area accessible to children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The facility will ensure that the cleaning compounds in the kitchen will be out of reach or latched where they are inaccessible.

Based on observation, the facility did not comply with the section cited above as 2 out of 2 play yards did not have tight fitted sheet for the size of the mattress which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The facility will ensure that the fitted sheets for the play yards are tight fitting for the mattress size.

Based on observation, the facility did not comply with the section cited above as 2 out of 2 play yards had sound machines and plastic arch attached and 1 out of 2 play yards had a balloon hanging above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The facility will ensure that there are no loose articles and objects hanging above or attached to the side of the crib.

Based on record review, the facility did not comply with the section cited above as the facility did not report all the adults living in the home which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The facility will ensure to update their license application to reflect all adults living in the home.
