Based on observation, interview and record review, the licensee did not comply with the section cited above for Assistant, S1 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to have Assistant complete the Mandated Reporter Training and submit certificate of completion to LPA by 09/05/2025.

Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed an air freshener in the children's bathroom within reach of children. LPA also observed cleaning product in lower cabinet in the laundry area and laundry detergents on the counter. LPA advised Assistant to move those items to high cabinets so they are inaccessible to children.

Resolution: Licensee to submit picture proof and written plan of correction to LPA by 09/05/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above for three (3) children (C1, C2 and C3) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to obtain immunization records for three children (C1, C2 and C3) and submit to LPA by plan of correction date of 10/03/2024.

Based on observation, interview and record review the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensee did not have an updated children's roster.

Resolution: Licensee to submit an updated roster to LPA by plan of correction date of 09/23/2024.

Based on observation, interview and record review, the licensee did not comply with the section cited above for 4 out of 4 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and staff will complete the online Mandated Reporter Training and provide proof of completed training to LPA by POC date of 08/07/2023.

Based on observation, interview and record review, the licensee did not comply with the section cited above for 3 out of 4 staff which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of TB test for two staff (S3 and S4) and proof of MMR immunization for one staff (S1) by POC date of 08/07/2023.

Based on observation, interview and record review, the licensee did not comply with the section cited above for 4 out of 7 children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee, Ravi Shah, will provide proof of immunization for four children by POC date of 08/07/2023.

Based on observation, the licensee did not comply with the section cited which poses a potential health, safety or personal rights risk to persons in care. Licensee did not have a working carbon monoxide detector.

Resolution: Licensee, Ravi Shah, purchased a carbon monoxide detector online while LPA was present.

Based on observation, interview and record review, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. Licensee has a uncleared adult (tenant) residing in the home.

Resolution: Licensee will provide a plan of correction to LPA by POC date of 07/25/2023.
